Purpose: To develop the physical therapy protocol based on the International Classification of Functioning, Disability and Health (ICF) framework for home-based rehabilitation following hip fracture surgery.  
Materials and methods: A three-round Delphi study was conducted. Health professionals were asked to score and provide recommendations regarding home-based rehabilitation following hip fracture surgery.  The level of agreement was considered at least 75%.
Results: Eight health professionals consist of two orthopedic surgeons, two physiatrists and four physical therapists. All participants completed three rounds of study. The final protocol domains included basic knowledge, assessment, progressive strengthening exercises, weight-bearing and balance training exercises, functional mobility training and education programs.
Conclusion: Comprehensive physical therapy home-based rehabilitation protocol following hip fracture surgery has been constructed in this study. This protocol was reached on consensus by health professionals and included all aspects of importance domains in hip fracture rehabilitation.
