ORIGINAL_ARTICLE Responding to the Needs of Aging Pupulation The phenomena of "graying" of population which was started in 20th century will continue in the next. It confronts the world with new challenges in the realms of health economy, politics, and cultural issues which are very difficult to overwhelm. The problems arising of this phenomena must be welcome, because they are indicators of upcoming development. Aging of population has occurred in developed countries since many years ago. Therefore their experience in dealing with the resulted problems will be of great help to the developing countries, but it must be remembered that every country has its unique needs and perspectives of future. Thus, modification of existing models of service delivery to elderlies in order to match with the specific conditions in each country is a necessity and major task for policy makers. Regarding these facts, United Nations in their second General Assembly on the topic of "Aging" rendered the International Strategy for Action on Aging as general guidelines, but stressed that every country must tailor programs to fit to its individual circumstances. In this article, an overview of future demographic changes of the world, challenges of the aging population, and the world programs on aging will be rendered and then a brief glance to what is happening in Islamic Republic of Iran as a country which is going to age in next few decades will be presented. http://irj.uswr.ac.ir/article-1-638-en.pdf 2005-09-01 10 17 Mahshid Foroghan 1 Iranian Research Center on Ageing, University of Social Welfare and Rehabilitation Sciences, Tehran, Iran. To analyse the data, descriptional statistics and Chisquare test were applied. Based on the results, 86% of the elderly are unofficially employed. Also, 95.7% of the questioned people are working after retirement and 7.2% have two jobs at the same time. 70.9% declared that their income is not sufficient for life expenses. 97.4% said that they are employed contract-based and temporary. 58.2% of them assumed that their main present problem is inability to cover the life expenses. Therefore, different needs of the elderly should be studied to improve their living status by providing them necessary support. ORIGINAL_ARTICLE Prediction of Risk of Falling among Institutionalized Elderly People in Iran Aging declines abilities and leads to increased risk of falling and subsequently poorer quality of life. The objective of this study was to identify a proper assessment for risk of falling among institutionalized elderly. In order to accomplish this aim, two functional assessment methods, Berg Scale and Tinetti Scale, were used and the validity indices of these methods were calculated. Prospective non-interventional methodological discriminative-validation study in order to make a comparative assessment of the discriminative validity of the two clinical assessment methods among the elderly people with/without history of falling. The frequency of falls within 6 months among institutionalized elderly individuals was prospectively studied. Finally, those having had two or more falls within 6 months were enrolled in the faller group and those having no falls within 6 months were enrolled in the non-faller group. Twenty-one women and 33 men (mean age: 75.79, standard deviation [SD]: 8.47, range: 61-98) independent in their daily activities and able to walk 10 meters using/without assistive devices volunteered to participate in the study with awareness. Background variables included age, gender, use/nonuse of assistive devices, height, weight, number of the drugs used, and number of the diseases; independent variable was history of falling; and dependent variables were the results of the two functional assessment methods. Independent t-test indicated a significant difference between the two groups of fallers and non-fallers in the mean scores on Berg Balance Scale (P=0.0001) and Tinetti Scale (P=0.0001). The results of logistic regression test indicated much more discriminative validity for Berg Balance Scale test than Tinetti Scale test. Studying the validity of Berg Balance Scale assessment method showed that all validity indices should be regarded as bases for clinical decision. http://irj.uswr.ac.ir/article-1-641-en.pdf 2005-09-01 37 47 Elderly people Fall(s) Functional assessment method(s) Ahmad Ali Akbari Kamrani 1 Department of Geriatrics, University of Social Welfare and Rehabilitation Sciences, Tehran, Iran. ORIGINAL_ARTICLE Development of a Community-Based Care System Model for Senior Citizens in Tehran In recent years a variety of system models for providing community based health care services for the elderly has been envisioned and implemented in the developed countries which meet the special care needs of different groups of elders. In Iran as a developing country, a considerable percentage of old people live in the society need to receive specialized care and health services. In order to respond to these care needs, developing and implementing health and social care systems with consideration of relevant factors such as: existing and available resources and facilities (financial and human resources); social and cultural issues and characteristics of each groups of elders(healthy elders, frails, home bounds, …)  seem to be necessary. At the development phase of the care model in this study Ellowing methodological triangulation must be considered: 1) Comprehensive review of current and related literature, 2) Conduction of an ethnographic study on a number of Tehranian elders and their families, 3) Seeking opinions of a group of experts on this issue using nominal group technique, and analysis as well as synthesis of the collected data were employed to develop a community based care system for elders. The preliminary results of employing this care system and examination of expected outcomes such as promoting quality of life and expectancy in elders reflects the efficiency of this system, although further complementary studies and particularly cost benefit analysis are strongly recommended. http://irj.uswr.ac.ir/article-1-642-en.pdf 2005-09-01 48 60 Health system model Community-based care Senior citizen Kian Norouzi 1 Iranian Research Center on Ageing, University of Social Welfare and Rehabilitation Sciences, Tehran, Iran.
