Objectives: Idiopathic Parkinson’s disease (iPD) and atypical Parkinsonian syndromes (APS) represent two major categories within the broader spectrum of Parkinsonian disorders, both of which may involve swallowing and cognitive impairments that affect quality of life (QOL). This study examined relationships among swallowing function, cognitive performance, and QOL and compared these outcomes between iPD and APS.
Methods: This cross-sectional observational study was conducted in spring 2025 at hospitals and neurology centers affiliated with Mashhad University of Medical Sciences, Iran. Ninety-seven individuals with Parkinsonian disorders participated. Swallowing, cognition, and QOL were assessed using the Mann Assessment of Swallowing Ability, Mini-Mental State Examination, and Parkinson’s Disease Quality of Life Questionnaire, respectively. Spearman correlations, Welch’s ANOVA, and hierarchical multiple regression were used to examine associations, diagnostic-group differences, and incremental contributions to QOL.
Results: Swallowing and cognitive inefficiencies were observed in 29.9% and 30.9% of participants, respectively. Swallowing function was positively associated with QOL (ρ = .59, p < .001), cognitive performance with QOL (ρ = .40, p < .001), and cognition with swallowing (ρ = .51, p < .001). Participants with APS demonstrated more inefficiencies in swallowing function, cognitive performance, and QOL than those with iPD. Cognitive performance initially explained 16.4% of QOL variance, and swallowing explained an additional 11.2%. Diagnostic group did not explain significant additional variance. In the final regression model, swallowing function remained significantly associated with QOL after accounting for cognitive performance and diagnostic group.
Discussion: Swallowing function, in addition to cognitive function, contributed to explaining the variance in quality of life, whereas the diagnostic group did not yield a significant improvement in the model. Given the cross-sectional nature of this study, these findings should be interpreted as "correlations" rather than "causal relationships."
